OverviewStepStone Family & Youth Services provides trauma informed, youth centered services to children and families involved in foster care and community based support programs. The Wraparound Facilitator plays a vital role in helping children and families access individualized supports and services that promote safety, stability, family preservation, and long-term wellbeing.
We are seeking a compassionate and family focused Wraparound Facilitator who is passionate about supporting youth and families through trauma informed, strengths based, and family centered practices. This role is ideal for a professional who values collaboration, advocacy, community engagement, and service coordination while helping families navigate complex emotional, behavioral, and environmental challenges.
The Wraparound Facilitator is responsible for coordinating services, supporting referral and intake processes, collaborating with treatment teams and community providers, and helping families identify strengths and resources that support successful outcomes. This position works closely with youth, caregivers, referral sources, and community agencies to ensure services are responsive, individualized, and focused on preserving family relationships whenever safely possible.

Competitive salaryMedical, dental, and vision insuranceGenerous paid time off and holidaysCompany paid basic life and AD&D InsuranceHealth Savings Account401(k) Retirement PlanSupportive and collaborative team environment Responsibilities Consults with Program Manager regularly and as neededResponsible for contract compliance and oversight of program policies and procedures Participates in quality improvement activitiesEnsures that documentation is thorough, accurate, and timelyEnsures that pertinent data is collected and complied as required Obtains and maintain proficiency certification to administer CANS assessmentsBuilds rapport and cooperation/service coordination with families and other community social service providersObtains needed reports and documents as needed from external sourcesPlans and facilitates Wraparound Team meetings of the youth, family, and service providersDevelops and implements individualized plans of care in collaboration with the Wraparound TeamIdentifies and develops formal and informal services/resources available to the youth, family, and communityConducts home/site visits with youth and/or family in accordance with the needs determined in the individualized Wraparound planTracks and monitors youth and family progress and adapt the Wraparound plan as needed and document all pertinent information in a timely fashionProduces written reports as neededMaintains client records in compliance with organizational goals and objectives, as well as external regulatory guidelinesIdentifies service gaps and collaborate with community stakeholders and existing service providers to resolve themResponds to client crisesAttends trainings as needed to ensure quality service provision and fidelity to wraparound modelOther duties as assigned Qualifications Bachelor’s degree in Social Work, Counseling, Psychology, or a related fieldTwo years of post –college work experience providing direct services to children and familiesPossess knowledge of mental illness diagnoses and behavioral disorders in childrenAbility to collaborate closely with a team of family members and professionals to develop and implement individualized plans of careFlexible with schedule in order to meet with youth and family wraparound team membersBilingual skills are preferred (Spanish) About our Line of Business StepStone Family and Youth Services, an affiliate of BrightSpring Health Services, provides the full spectrum of support to children in need of alternative, safer, and more positive living environments with residential and family services.
