As a Wraparound Facilitator, you'll provide high fidelity wraparound services, promote family voice and choice, and collaborate with key stakeholders and community partners. Our ideal candidate is organized, communicative, and committed to supporting youth with serious emotional disturbances (SED) and their families.

Key Responsibilities:

Facilitate high fidelity wraparound services using current policies and procedures

Collaborate with site coaches and supervisors to implement feedback

Develop organizational systems to manage DMHA and other funding requirements

Ensure timely and accurate documentation in compliance with ethical standards

Build and maintain internal and external partnerships to support client needs

Communicate with community groups, funders, courts, and other stakeholders

Manage assessments, service approvals, EMR entries, and follow-ups

Understand and manage various funding sources and covered services

Assess risk and develop safety plans; report abuse/neglect and emergencies

Coordinate acute care admissions and placements (PRTF/RTF/SOF)

Meet assigned metrics including documentation timeliness and compliance

Facilitate and bill for Child and Family Team Meetings (CFTMs)

Participate in staff development and contribute to program operations

Other duties as assigned by Program Leadership
 

Education and/or Experience Qualifications:

Bachelor’s degree required

Minimum of 2 years of experience working with SED population or under clinical supervision by a licensed provider
 

Required License/Certification (one of the following):

None required
 

Additional Qualifications:

Strong organizational and time management skills

Excellent oral and written communication skills

Ability to present to large groups

Customer service oriented

Ability to engage youth, families, and stakeholders

Ability to work independently and as part of a team

Proficient in Microsoft Office
