Home Health WOCN RN

$10,000 Bonus

Territory: Maywood, Oak Park, Forest Park, Melrose Park, Elmwood Park, Schaumburg, Carol Stream, and Chicago zip codes north of North Ave.

Position Purpose: The Home Health WOCN RN provides expert nursing care in wound, ostomy, and continence management to home care patients. They collaborate with healthcare professionals and patients/families to ensure optimal outcomes.

Qualifications:

Graduate of an accredited nursing program (BSN preferred)Current RN license in the state of practiceCertified Wound Ostomy Continence Nurse (CWOCN) certification requiredMinimum 2 years of RN experience in a clinical settingProficiency in electronic medical records systemsStrong assessment, critical thinking, and communication skillsAbility to work independently and as part of a teamValid driver's license and reliable transportationCurrent CPR/BLS certification

Essential Functions:

Conduct comprehensive assessments of wounds, ostomies, and continence issuesDevelop individualized care plans with the interdisciplinary teamEducate patients and families on wound care, ostomy care, and continence managementProvide direct patient care and monitor progressParticipate in quality improvement activities and maintain documentationProvide in-service education and participate in continuing education
