Job Description
We are seeking a dedicated Wound Care Nurse to join our team, committed to providing exceptional care to patients requiring wound management. This role involves assessing and treating various types of wounds, developing individualized care plans, and collaborating with an interdisciplinary team to ensure optimal patient outcomes.
ResponsibilitiesConduct comprehensive assessments of wounds including pressure injuries, surgical wounds, diabetic ulcers, venous/arterial ulcers, and other skin breakdowns.Develop and implement individualized wound care treatment plans in collaboration with the patient’s physician.Provide direct wound care such as debridement, packing, dressing changes, and use of advanced wound products.Monitor wound healing progression and document in the HCHB system.Educate patients and caregivers on wound care, infection prevention, nutrition, and self-care techniques.Collaborate with the interdisciplinary team including physical therapists, occupational therapists, and case managers.Communicate changes in patient condition or treatment needs promptly to the physician and care team.Maintain accurate, timely, and detailed clinical documentation.Follow infection control procedures and universal precautions during all care activities.Perform OASIS assessments, SOC, ROC, and other assessments as needed.Essential SkillsActive Connecticut RN License.Minimum of 1 year of wound care experience.Wound care certification (WCC, CWOCN, or equivalent) - required.Familiarity with home health regulations, OASIS documentation, and HCHB.Additional Skills & QualificationsTime Management & Autonomy: Ability to manage a caseload independently and prioritize patient visits efficiently.Patient Education & Advocacy: Skill in educating patients and families about care plans, medications, and recovery strategies.Technology Proficiency: Comfort with mobile documentation tools, EMR systems, and telehealth platforms.Cultural Competency: Ability to provide respectful and effective care across diverse populations.Crisis Management & Adaptability: Responding calmly and effectively to urgent changes in patient condition or environment.BSN Degree: Preferred but not required.Experience with Chronic Disease Management.Certification in Wound Care or IV Therapy.HIPAA Compliance Knowledge.Work Environment
The work schedule is from 8:30 AM to 5 PM, with any travel limited to the surrounding towns of Greenwich in Fairfield County. Pay and Benefits
The pay range for this position is $100000.00 - $110000.00/yr.
