Job Summary
The Hospice RN Weekend Case Manager provides skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice. The Hospice RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling in accordance with the interdisciplinary team care plan.

Education
Other Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferred
Licenses and Credentials
Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only required Registered Nurse [RN - State License] - Generic - HR Only required
Experience
Minimum of one (1) year medical/surgical nursing experience required Hospice/home care experience preferred
Knowledge, Skills and Abilities
- Graduate of an approved, accredited school of professional nursing required.
- Demonstrated excellent verbal and written communication skills required.
- Demonstrated strong interpersonal, organizational, and time management skills required.
- Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required.
- Good driving record, and reliable transportation required.
