Job Summary
Under the direction of the Clinical Manager and within established organizational and departmental policies and procedures, responsible for providing environmental, personal, and health care services that contribute to client/family health, well being and safety. Work under the supervision of a Registered Nurse or Therapist and is supervised at least every sixty-two (62) days for personal care while providing the care or every fourteen (14) days for direct health care services. HCA’s are assigned to specific patients by the staff schedulers or other appropriate professional staff.
Qualifications
Successful completion of a HCA certification program and a written skills test and competency evaluation required.Minimum of six (6) months work experience in a supervised setting required, preferably home care agency or other health care facility.Demonstrated ability to read, write and follow a written Plan of Care required. Demonstrated good interpersonal and communication skills required. Demonstrated ability to be self-directed and the ability to work with only periodic supervision required. Demonstrated dependability, tact, patience and the ability to follow instructions required.Current valid driver’s license, good driving record and reliable transportation required.
