Responsibilities Include:

Interact with Atlantic Visiting Nurse team members in an office and community setting Working hand-in-hand with our visiting nurses to see patients in their home environment and help them towards their individual health goals Working with our specialty programs, including Remote Patient Monitoring, Virtual Care, and Wound Program, which provide skilled consultations; these programs aim to reduce the risk of patients being re-hospitalized, while providing an elevated level of assessment and collaboration Working with our office team members, who support the patient experience and provide daily oversight to workplace operations Includes positive collaboration with an interdisciplinary team to report changes in the patient’s condition and progress towards goals
