Job Description
This position is centered on care management with a strong emphasis on transitions of care, including coordination and medication reconciliation. The primary goal is to reduce emergency department visits and hospital readmissions. The role involves handling all inpatient discharges, excluding labor and delivery, and oncology follow-ups, as well as focusing on emergency department visits, particularly for patients with chronic conditions.
ResponsibilitiesCoordinate transitions of care for patients to reduce emergency department visits and readmissions.Perform medication reconciliation and ensure accurate medication management.Manage all inpatient discharges, excluding labor and delivery and oncology follow-ups.Focus on emergency department visits with an emphasis on patients with chronic conditions.Essential SkillsCase management experience.Proficiency in EPIC software.Active RN license in the state of Washington.Experience in care coordination.1+ years of experience in case management within an acute hospital, long-term acute care, rehab, or skilled nursing setting.Additional Skills & QualificationsBasic Life Support (BLS) certification.Experience with utilization review and discharge planning.Knowledge of population health and interqual.Work Environment
The position is based in an outpatient clinic, with a hybrid schedule of 2 days per week on-site at the Beacon Hill clinic and 3 days per week working from home. The clinic will provide all necessary equipment for remote work. Working hours are 8:00 AM to 4:00 PM PST, Monday through Friday.

Pay and BenefitsThe pay range for this position is $40.00 - $60.00/hr.
