In an Olathe Health Urgent Care Clinic, the APP practice model includes providing care for low, mid, and high acuity urgent care same-day walk-in patients of all ages with various acute and chronic health concerns.
Responsibilities and Essential Job Functions
Collaborates appropriately with sponsoring/supervising physician in clinic when possible, or virtually or telephonically when necessary, and does so in a timely manner to ensure thorough and exceptional patient care.Maintains clear and accurate patient documentation within 48 hours of the patient visit.Consistently adheres to provided work schedule.Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.Whenever necessary to ensure best practice of evidence-based medicine, discusses case with sponsoring/supervising physician and/or other medical professional to prepare comprehensive patient care plan.Maintains open and clear communication with sponsoring/supervising physician(s).Ability to confidently assess patient and formulate an appropriate treatment plan.Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patients for consultation and continued medical care (PCP, specialty provider, outside healthcare provider) as appropriate.Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care.Works in an independent and interdependent relationship with members of the medical staff to allow for consultation, collaboration, or referral.Responsible for the diagnosis and treatment of acute, chronic, and long-term health care issues.Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.Performs physical examinations and preventive health measures within prescribed guidelines and scope of practice determined and agreed upon by both the APP and sponsoring/supervising physician.Participation in group activities, meetings, projects.Adheres to written protocols specific to position.Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities.
