You'll become a trusted resource across the region while helping ensure patients receive exceptional hospice care—wherever they call home. **Bring your clinical expertise on the road and make an impact across Western Pennsylvania.** **As a Regional Support Hospice RN, You Will:** * Deliver high-quality, compassionate nursing care for hospice patients * Manage and implement individualized plans of care * Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.) * Assess and address patients’ physical, emotional, spiritual, and environmental needs * Accurately document all care provided and ensure compliance with hospice protocols * Educate families and caregivers on care instructions, medications, and safety * Participate in the on-call rotation and provide after-hours care as needed * Collaborate with physicians, referral sources, and the care team * Assist with referrals, regulatory documentation, and payer compliance * Travel to various Gentiva hospice locations as assigned to fill urgent needs **Schedule & Travel:** This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities.
