Transplant Coordinator - Blood and Marrow Transplant and Cellular Therapy

The role of the Transplant Coordinator at Cleveland Clinic's Taussig Cancer Center is to coordinate the care of patients seeking candidacy and those in receipt of solid organ(s) (heart, lung, heart-lung, liver, kidney, pancreas, vascularized composite allograft and multi-organ) and bone marrow for transplant. In collaboration with members of a multidisciplinary team, the Transplant Coordinator will enhance patient care by developing relationships through education and service and will serve as an integral member with a goal to deliver exceptional care to a specific population of patients experiencing end-organ failure or disease. As a Transplant Coordinator in Blood and Marrow Transplant, you will organize all the programs and activities for our transplant patients, provide education on the transplant process to patients and their families and will be responsible for understanding proper treatments plans. **A caregiver in this role works days from 8:00 a.m. - 5:00 p.m.** A caregiver who excels in this role will: + Coordinate the care of patients seeking candidacy and those in receipt of solid organ(s) (heart, lung, heart-lung, liver, kidney, pancreas, vascularized composite allograft and multi-organ) and bone marrow for transplant. + Enhance patient care by developing relationships through education and service. + Monitor, record, and communicate patients' conditions as appropriate. + Serve as an integral member with a goal to deliver exceptional care to a specific population of patients experiencing end-organ failure or disease. + Present the potential transplant candidate or the transplant recipient to the transplant physician and document the need for additional assessment or intervention as determined by the care team. + Provide education to patients and caregivers with specific focus on a single phase of the transplant process to provide optimal outcomes following transplant. + Facilitate the scheduling of studies or procedures utilizing fiscal responsibility for patient and hospital costs. + Communicate with the referring physicians on the plan of care established for the patient in collaboration with surgical and medical teams. + Complete required documentation according to Organ Procurement Transplant Network (OPTN) guidelines including listing status changes and post-transplant follow-up information as applicable. + Review and collate medical information received from referring physicians for patients seeking or undergoing transplant evaluation. + Transfer communication concerning transplant patient for peri-transplant evaluation and post-transplant care for physician and patient/caregiver and/or payors seeking information. + Coordinate all activities related to organ acquisition including evaluation of the patient workup information and/or post-transplant care. + Be available by pager and by phone to provide immediate assessment and delivery of care in a timely and cost-effective manner. Minimum qualifications for the ideal future caregiver include: + Basic Life Support (BLS) certification + Current state licensure as a Registered Nurse (RN) + Three years of acute care hospital experience including disease related experience Preferred qualifications for the ideal future caregiver include: + Bachelor of Science in Nursing (BSN) required within three years of hire per Cleveland Clinic policy + Oncology experience Our caregivers continue to create the best outcomes for our patients across each of our facilities.
