Reporting to the Senior Manager, the Telehomecare Clinician provides remote patient monitoring, health coaching, and disease-specific education to patients. The Telehomecare Clinician will remotely monitor and coach patients with complex chronic diseases. This role is based on a collaborative model of care that focuses on enabling the patient to better manage his/her chronic disease through the support of clinician monitoring and coaching. The Telehomecare Clinician will work in collaboration with the patients’ circle of care.
Please note that the Telehomecare Clinician will be responsible for both remote care and remote surgical monitoring.
What will you do?
Conduct eligibility for any referred patientsObtain informed patient consent to participateCreate and implement a customized care plan with patients that best meet the patient needsAssist patients in establishing and implementing self-management goalsSupport patients in medication awareness and managementMonitor patients and respond to alerts using established clinical protocols and guidelinesRespond to patient phone/email inquiries and determine clinical next stepsDeliver patient education using validated educational tools and resourcesConduct regular and just-in-time health coaching sessions with patientsRemotely monitor and manage an assigned patient case loadActively participate in ongoing training/education sessions and Community of Practice offered through the Telehomecare CentreParticipate as a member of the Telehomecare Team to provide feedback into continuous quality improvement initiativesParticipate in program evaluation activities when requiredConduct home visits and face to face assessments where required.Participate as a member of a selected number of committees representing Telehomecare issuesEmbody Ontario Health atHome mission, vision and values and apply quadruple aim (enhancing patient experience, enhancing provider/staff experience, improving value and improving populations health) to support continuous quality improvement in daily workExemplify, embrace and intentionally promote an inclusive work environment where all are meant to feel they belong
What must you have?
University degree preferred in Nursing, and /or equivalentHolds current appropriate unrestricted registration as a member of the College of Nurses of OntarioExperience in patient centered models of care where patient experience is a priorityExcellent knowledge and expertise working with complex chronic diseases including, but not limited to: CHF, COPD, palliative care, mental health, chronic kidney disease and diabetesFoundational knowledge in clinical practice, decision-making, critical thinking, leadership, research utilization, resource management and problem solving skillsProficient with adult health education delivery, health coaching techniques and patient self-management support Protect patient confidentiality in accordance with legislation and college requirements
What would give you the edge?
Completion of Respiratory Therapy program Other relevant certifications (i.e. CDE, CRE, CNA designations, TEACH, Choices & Changes, etc.) ICU, Cardiology Unit, or ER experience Experience with motivational interviewing counselingAbility to speak French or another second language
Hours of work:
All hours of operation (Initially Monday-Friday 0830-1630; 70 hours per bi-weekly pay period) subject to change as per the Collective Agreement.
Position location and travel:
Hybrid work model, subject to change.
Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
