Team Lead-Disease Burden Capture - Population Health Value Based Care Operations

Oversee daily operations and hold team members accountable to the expectations of the assigned jobs. Lead the team in understanding and ensuring work is following standardized guidelines and regulations, identify areas of process improvement, educate team when needed, including new processes and execution and hold regular meetings with the team to review expectations and needs of the team. Work with management to ensure all clinical aspects necessary to meet team goals are in place. This position is a working supervisor and will also perform daily work in support of departmental and organizational goals, as needed **ESSENTIAL DUTIES:** The percentage of time spent performing essential functions is 95%.
