Position Summary:

Reporting to the Stroke Coordinator, the Nurse Navigator provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members and support services. Coordinates multidisciplinary care based on evidence based practice. Responsible for providing stroke education, participation in stroke related research, and delivery and oversight of patient care including post discharge follow up. Works in collaboration with Stroke Program key stakeholders to meet stroke program certification requirements, including review of data and performance improvement projects. Incorporates the patient experience in all aspects of patient care.

Position Qualifications    Minimum Education:Associates Degree in Nursing required; Bachelors Degree preferredMinimum Experience/Skills:Minimum two years of neuroscience experience required.   Analytical and grammatical skills are necessary to communicate effectively, verbally and in writing.   Must be fluent in spoken and written English.  Must be able to fulfill the essential functions of the position.License/
Certificates/
Credentials:Valid California RN license. BLS and ACLS. Current National Institutes of Health Stroke Scale (NIHSS) Certification or completion within three (3) months of hire. Current Advanced Stroke Life Support (ASLS) Certification or completion within three (3) months of hire. 
Preferred Neuroscience or Stroke Certification such as Stroke Certified Registered Nurse (SCRN), Certified Neuroscience Registered Nurse (CNRN) or Neurovascular Registered Nurse (NVRN).                                                                                                                                                       Physical Demands:Prolonged, extensive or considerable standing/walking. Lift/position up to 50 lbs. Push/pull up to 50 lbs of force. Frequent reaching, stooping, bending, kneeling, crouching. Manual dexterity and mobility
Ability to move quicklyVision Requirements:Ability to adjust vision sufficiently to perform position accountabilities.Working Conditions:Regularly exposed to the risk of contagious and blood borne diseases. Subject to varying and unpredictable situations. Exposure to unpleasant elements (accidents, injuries, illness). Emergency and crisis situations. Subject to irregular hours. Increased stress due to multiple calls and inquiries
Occasional exposure to radiation hazards, radioactive substances, and biohazards. Outpatient, Radiation Oncology, Operating Room (limited) and Inpatient environment. Frequent exposure to hazardous chemicals, sick patients, bodily substances, extreme noise, radiation, lasers, etc. Requires  lifting, assisting, positioning of patients, handling of instruments/equipment, etc.
