Job Summary:\rResponsible for integrating patient-centered pharmaceutical care with traditional distributive and drug control activities. Provides consultative/clinical services including antibiotic dosing of vancomycin and aminoglycosides, initial warfarin dosing, renal dosing, and IV-to-PO conversions. Requires strong clinical and communication skills for providing drug information to nurses, physicians, and other health care providers. Capable of advising the patients' medical team on drug interactions, contraindications, and other relevant information to ensure proper, accurate, and optimal medication therapy. Responsible for adhering to and practicing within the guidelines of Georgia State Pharmacy Law.\rCore Responsibilities and Essential Functions:\rGeneral Pharmacy Practice\r* Preparing IV Admixtures, compounding, and repackaging a wide variety of pharmaceuticals\r* Entering and verifying orders into the pharmacy computer system, and proper dispensing of medications\r* Able to follow patients for antibiotic and anticoagulant dosing\r* Participates in daily rounds\r* Works with other healthcare providers to assist in the patient education for new medications and their side effects\r* Assists with medication reconciliation and discharge counseling\rKnowledge of drug appropriateness\r* Evaluating and monitoring drug regimens for appropriateness.\r* Interpreting physicians orders and screening them for various types of interactions.\r* Providing patient and nursing education when requested.\r* Reviewing medication profiles to maintain accuracy and optimize medication therapy.\r* Able to perform all duties of a Pharmaceutical Care Pharmacist\rEducation Skills\r* Providing supervision and guidance to all personnel working in their area\r* Demonstrating skill and competency in patient assessment, and providing patient and nursing education when requested.\rPerforms other duties as assigned\rComplies with all Wellstar Health System policies, standards of work, and code of conduct.\rRequired Minimum Education:\rBachelor's Degree B.S. from an accredited School of Pharmacy.\rDoctorate Pharm.D. from an accredited college of pharmacy\rOther Education Completion of a PGY1 Pharmacy Residency\r\rRequired Minimum License(s) and Certification(s):\rAll certifications are required upon hire unless otherwise stated.RPH - Reg Pharmacist\rAdditional License(s) and Certification(s):\rBCPS: Board Certified Pharmacotherapy Specialist preferred Upon Hire Required\rRequired Minimum Experience:\rHospital pharmacy experience Preferred and\rMinimum 2 years clinical pharmacy experience Preferred\rRequired Minimum Skills:\rStrong order entry and computer skills\rGood oral communication skills\rAbility to think/act independently as required by situation\rAbility to collaborate with other professionals to ensure good patient experience
