Position Summary:

Person in this position will be responsible for providing intensive, person-centered case management services to veterans and service members who are identified as at-risk for suicide. This role focuses on addressing immediate needs, reducing barriers to care, and coordinating services to support stabilization, recovery, and long-term wellness. The Case Manager works closely with the intervention and peer support teams to ensure participants receive timely and appropriate resources, including mental health care, substance use treatment, housing, employment support, and other wraparound services. Essential Job Functions include but not limited to those listed below.

Essential Job Functions: Conduct comprehensive assessments to identify each participant’s strengths, needs, and risks. Develop individualized service plans focused on reducing suicide risk and enhancing protective factors. Provide ongoing case management, including coordination of mental health, substance use, housing, and other social services. Support crisis intervention efforts in collaboration with intervention specialists and peer support staff. Conduct regular follow-up with participants to monitor progress, adjust care plans, and ensure service continuity. Serve as a liaison between participants and service providers, ensuring warm handoffs and effective communication. Maintain accurate and timely documentation in compliance with grant requirements and confidentiality standards. Participate in multidisciplinary case conferences, trainings, and team meetings. Assist with tracking grant outcomes and preparing the required reports.
