Job Summary:\rThe Speech Language Pathologist (SLP) is responsible for age appropriate interventions for the client that meet physical, emotional, developmental, psychosocial and educational needs to patients whom require rehabilitative care. The Speech Therapist is responsible to collaborate with all disciplines throughout the episode of care, to ensure all goals are met. The SLP provides delivery of high quality continuous care providing assessment, therapeutic exercises, education, treatments and self-maintenance therapy in the patient's home. The Therapist participates in a preceptor role, for new employees and works in cooperation with other disciplines. The Therapist is responsible for attending inservices, staff meetings, special projects and continuing education.\rCore Responsibilities and Essential Functions:\rDirect Patient Care\rAccess and manage patients rehabilitative needs utilizing general principles governing reasonable and necessary therapy.\rManage patient from admission through discharge and appropriately schedule visits per MDs order. Adjust frequency as needed based on status and MD order\rPerform routine speech therapy procedures to meet the need of the patient.\rMeet established productivity standards.\rSeek training, supervision or direction from leadership as needed\rCoordination of Services\rDevelop the patients plan of care and revise it based on patient need within MD orders\rCoordinate the care and services with the Clinical Supervisor and other team members and support services.\rPerform other duties as assigned by Home Health Leadership.\rMaintain communication with MD reporting any changes that may affect the patients recovery.\rCommunication and Documentation\rResponsible for accurate completion of skilled intervention documentation within established timeframe.\rCommunicate patient status changes to the MD\rDemonstrate knowledge and appropriate application of regulatory guidelines\rParticipate in care team conferences with coworkers\rResponsible for checking voice mail box and secure messaging a minimum of once daily.\rPatient/Caregiver Education\rServe as patient educator to foster care or caregiver support for the home patient.\rBe aware and report concerns to authorities or agency leadership on any issues that affect patient or staff safety\rUtilize both verbal and written tools to facilitate learning and promotion of independence during recovery.\rREGULATORY GUIDELINES\rDemonstrates knowledge and appropriate application of CMS/State, and Joint Commission regulatory guidelines.\rResponsible for accurate completion of visit documentation in the patients EMR\rCompletes all documentation within established timeframes\rStaff Education and Professional Development\rParticipate in the orientation and education of new employees as appropriate.\rResponsible for attending continuing education and in-services as directed and needed.\rAccept responsibility to remain current in clinical knowledge for the care of the home health patient.\rPerforms other duties as assigned\rComplies with all WellStar Health System policies, standards of work, and code of conduct.\rRequired Minimum Education:\rMaster's Degree Graduate of an accredited school of professional Speech Language Pathology.\r\rRequired Minimum License(s) and Certification(s):\rAll certifications are required upon hire unless otherwise stated.BLS - Basic Life Support or BLS-I - Basic Life Support - InstructorSLP - Speech Language PathologistGADL - Georgia Driver's License or VSDL - Valid State Driver's LicensePVI - Private Vehicle insurance\rAdditional License(s) and Certification(s):\rRequired Minimum Experience:\rMinimum 1 year Home Health experience Required\rRequired Minimum Skills:\rStrong critical care assessment and rehabilitative skills required.\rPositive leadership, problem solving skills, strong organizational and time management skills.\rStrong written and verbal communication skills.\rUse of window based computer, utilizing home health patient software, email, internet, computer based learning modules, etc.\rGood understanding of home health operations, reimbursement, law and regulatory processes.
