their clients and their clientsrsquo familiesThe Speech Language Pathologist independently evaluates patientrsquos level of function and determines develops and implements individualized treatment goals and plans in alignment with the patientcaregiver goals Determines treatment strategies and delivers Speech Language Therapy services that identify restore and rehabilitate disorders of communication cognition and swallowing to maximize a patientrsquos safety and independenceThis role encompasses evaluation skills and techniques analysis of patients and application of speech therapy to maximize the patients functional independenceUpon Physician referral performs thorough evaluation of patientrsquos oral motor strengthagility motor speech voice deglutition auditory comprehension reading comprehension verbal expression written expression memory problem solving and attention Evaluates the existence nature and severity of speech language andor hearing disordersRecognizes physiological psychological and age related changes in the patient Consults with other Health Care Team members as necessaryDevelop an individualized patient centered treatment plan of careEvaluate care plan on an on going basis revising as necessary in accordance with patientrsquos care needs Establish measurable goals with the patientfamily based on appropriate patient care protocols and critical paths in order to meet patient needsNotify Physician of any changes in status or progressDeliver therapeutic techniques that are appropriate to patientrsquos needs and medical statusImplement deliver and delegate patient care by utilizing appropriate interventions and age specific interventions and treatments through selected activities and exercisesProvides patient and caregiver education regarding treatment precautions and equipment useCompletes all documentation including but not limited to evaluation care plan and goals patient status and progress visits patient discharge summary and patient care transition plan and summaryRequires the timelyand accurate documentation and coding of skilled and medically necessary services provide to the patientParticipate in patient transition planning andor patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care Help to coordinate patient care with other care Speech PathologyExperiencePreferred Previous experience as a Speech Language PathologistCurrent State of Connecticut license in Speech Language Pathology or meets minimum requirements for a Supervised Professional Experience SPE plan required Certificate of Clinical Competence with passing score on the national SLP Praxisreg exam or Approved Clinical Fellowship plan from the American Speech Language Hearing AssociationBasic Life Support BLSBLS upon hire or within days of hire varies by facilityFor Home Care Only Valid driverrsquos license and reliable transportation requiredWe take great
