Job Details Our Home Health Speech Language Pathologists will: + Assess, plan, develop and provides the patient or responsible caregiver a home health plan of care for speech language pathology. + Identifies patient needs for other disciplines and services. + Coordinates and evaluates the plan of care with all other disciplines and physicians for an assigned caseload. + Instructs the patient or responsible caregiver on the projected care plan and expected outcomes as directed by the physician's treatment plan. + Detailed documentation of visits in EMR system. Minimum Qualifications: + Licensed and current registration to practice as a Speech Language Pathologist in New York State required. + Minimum of one year post-licensure experience in rehabilitation with adult speech/language impairment populations. + Good computer skills - Electronic Medical Record experience required. Salary Range USD $90,000.00 - USD $125,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
