Job Details The Social Worker will play a vital role in promoting employee well-being in Catholic health and in the wider community as part of the Workforce Resilience Institute of Nassau County. This position focuses on providing support, resources, and counseling to employees, fostering a healthy workplace environment, and acting as a navigator for patients within our systems. Duties include addressing opioid treatment and prevention strategies, coordinating internal referrals, and ensuring smooth transitions for patients as they shift to the next level of care. The Social Worker will help to foster a healthy workplace environment that reflects our commitment to compassion, integrity, and service. Key Responsibilities: Conduct assessments to identify the emotional, social, and psychological needs of employees and provide appropriate interventions or referrals. Act as a navigator for patients, in order to connect them with community-based organizations (CBOs) and ensure continuity of care. Assisting with transitions to beds and/or discharges from the hospital. Develop and implement programs aimed at enhancing resilience, work-life balance, and overall wellbeing within the workforce. Provide individual counseling and facilitate workshops that address stress management, coping strategies, and personal development. Collaborate with multidisciplinary teams, including HR, management, and employee assistance programs, to create supportive workplace initiatives. Maintain up-to-date knowledge of local and national resources available for employee support, including EAP, mental health services, legal assistance, and community services. Help with training sessions to educate staff about mental health awareness, crisis intervention, and resources available within the organization. Adhere to ethical standards and ensure confidentiality in all aspects of social work practice. Monitor and evaluate the effectiveness of employee well-being initiatives and make recommendations for improvement. Support the mission of the healthcare system and the Resilience Institute by integrating Catholic values into all aspects of social work and employee care. Qualifications: Master’s degree in Social Work (MSW) from an accredited institution. Active and unrestricted license to practice social work in the state of New York. Experience in mental health counseling, employee assistance programs, or similar settings preferred. Knowledge of workplace dynamics, including the impact of stressors on employee productivity and health. Excellent interpersonal and communication skills, with the ability to build rapport with diverse patients and employee groups. Strong problem-solving skills and ability to work collaboratively in a team environment. Commitment to the mission and values of a Catholic healthcare system, with sensitivity to spiritual and ethical considerations in care. Salary Range USD $65,000.00 - USD $65,000.00 /Yr.
