Social Worker - St. Vincent Medical Center
Job Summary
The Social Worker completes in-depth assessments of patients in order to evaluate candidacy for solid organ transplant. The associate provides counseling, support, and community resource referrals to aide patients/families in preparing for transplant. The associate follows patients pre-, peri- and post-transplant to assist with resolving psychosocial issues and lifestyle changes that improve a patient’s post-transplant success. The associate works closely with multi-disciplinary transplant team to assess inpatients in order to identify appropriate discharge plans. The associate provides assessment and intervention planning with patients experiencing substance abuse and/or mental health issues. The associate acts an advocate for patients that may be victims of Child Abuse, Domestic Violence, or Adult/Elder abuse. The associate will participate in community outreach and patient education related to transplant. The associate will assist manager in policy development, problem solving, program development, and staff training. The associate performs all other duties as assigned.
Essential Job Functions
Provides evaluation and intervention for transplant patients in a crisis situation.Provides short-term counseling/treatment to patients and families throughout the transplant process, adjustment to chronic illness, and/or hospitalization.Collaborates with multi-disciplinary transplant team to assess and formulate appropriate discharge plans for inpatients.Works to link transplant patients with Medication Assistance Programs when needed and facilitate other appropriate community referrals.Executes documentation and reporting requirements. Works closely with patients and staff to formulate treatment plans to resolve substance abuse, mental health, adherence, and/or psychosocial issues.Collaborates with multi-disciplinary transplant team on any/all regulatory issues related to transplant.
Provides other operational assistance as requested.

Licensing/Certification
Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) (required)
Education
Master of Social Work or Master of Science in Social Administration (required)
Work Experience
3 years in healthcare, social or human services setting. (required)
