Responsible for counseling, assessing, and assisting patients and their families who are affected by illnesses at the end of-life.

Principal Accountabilities:

1. Provides psychosocial assessments to determine patient needs, makes appropriate referrals, provides counseling and supportive services to patients/families. 
2. Maintains statistical information and analysis of data, develops outcome and measures for program evaluation, program development and facilitation of program. 
3. Other relevant duties as assigned
