All while understanding that **together** we are even better. **Schedule:** PRN, as needed **Shift** : Days 8am-5pm with weekend availability **Location:** Arapahoe, Denver, Douglas, Elbert, Adams, Jefferson Counties **The role you'll contribute:** Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community. **The value you'll bring to the team:** + Assess patients physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data. + Evaluates patients response and intervenes to ensure optimal patient outcome. + Develop and implement patient plan of care and observe outcomes in accordance with social + work standards and in collaboration with the interdisciplinary team. + Demonstrate competency in a variety of therapeutic/diagnostic interventions. + Promote, advocate and collaborate to protect the health and safety and rights of each patient. **The expertise and experience you'll need to succeed:** **Minimum qualifications:** + Master's degree + 1 year experience in a hospital setting + Active American Heart Association BLS + Clinical Social Worker License (LCSW) This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
