**Job Summary and Qualifications**
The Social Worker is responsible for the identification of and planning for psychosocial needs of patients, families and significant others to include the coordination of acute / post-acute services, discharge planning, and appropriate resource utilization within the hospital and the community. The Social Worker facilitates the interdisciplinary team to plan, implement and evaluate an individualized plan of care based on identified needs and available resources. The Social Worker implements patient safety activities to include: Actively seeks ways to control costs without compromising patient safety, quality of care or the services delivered; Demonstrates knowledge of the occurrence reporting system: uses system to document and report potential patient safety issues; Adheres to established guidelines for reporting a significant medical error or unanticipated outcome in the patient’s care which results in patient harm; Attends in-service presentations and completes mandatory education requirements including, but not limited to, infection control, patient safety, quality improvement, MSDS, and OSHA standards. The Social Worker advocates for patients within the guidelines of patient rights and policy and procedures of Valley Regional Medical Center. He/she supports organizational goals by providing quality customer service and participating in performance improvement efforts. The Social Worker demonstrates a commitment to teamwork and cooperation. The Social Worker will apply the mission, vision, and value statements of Valley Region Medical Center in the fulfillment of social service functions and comply with hospital policies and procedures
. **What you will do in this role:**
+ You will work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
+ You will identify and address barriers to discharge and recovery. These barriers include financial, emotional, psychosocial, medical, and spiritual.
+ You will maintain a thorough working knowledge of the rules of Medicare, Medicaid, and private payer regulations and processes. You will possess a thorough understanding of managed-care concepts and length-of-stay management relating to patient and organizational fiscal responsibility.
+ You will perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
+ You will coordinate and facilitate meetings between the care team and patients/families/caregivers. You will assist patients and families in making difficult decisions while honoring their preferences and values.
+ You will intervene with patients/families/caregivers for psychosocial needs, assistance, and support in coping with chronic acute terminal illness. You will engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
+ You will provide emotional support to patients and family members during complex and/or emotionally charged issues, such as end-of-life, terminal illness notifications, death notification (with physician), and crisis interventions.
**What qualifications you will need:**
+ Master’s Degree in Social Work with current Texas license
+ Ability to take call evening, nights and weekends
+ Two years’ experience in Social Work, preferred
