UPMC is offering a $6000 sign on bonus with a 2 year work commitment Responsibilities: + Provide resource-specific information and facilitate referrals for patients and families. + Mentor social work students and serve as a field instructor for MSW students. + Utilize therapeutic counseling methods, including crisis intervention and psychosocial support. + Educate patients and families on reimbursement systems and advocate for continued care authorizations. + Assess and intervene with medically and psychosocially complex patients. + Monitor and reassess the plan of care throughout the hospital stay. + Collaborate with the healthcare team and community resources to develop individualized care plans. + Educate and advocate for patients and families on medical care decisions, discharge planning, and end-of-life care. + Complete comprehensive psychosocial assessments considering various factors and support systems. + MSW degree from a CSWE accredited School of Social Work required. **Licensure, Certifications, and Clearances:** + Current / active Social Work License required or successful pass of the social work licensing exam within 9 months of employment. + Clinical Social Worker (CSW) OR Licensed Social Worker (LSW) + Act 31, Act 33, Act 34, Act 73 FBI **UPMC is an Equal Opportunity Employer/Disability/Veteran**
