Position Summary

Facilitation, coordination and communication between patient and family with referral sources to maximize patient's functioning capacity in the community. Continually exercises independent judgment and discretion, and within agreed upon limitations, makes patient management decisions involving interventions based on knowledge of the organization, policies, procedures, and professional code of ethics. Performs under minimal supervision.


Position Specific Responsibilities

• Assess hospice patients’ psychosocial, financial, environmental, and community needs, as documented in the initial social work assessment, Plan of Care (POC), clinical records, and team meeting notes.

• Provide direct and indirect social services support for patients and families in all HOL service settings (homes, long-term care facilities, hospitals).

• Deliver crisis intervention and supportive counseling for patients and families adjusting to death, dying, and terminal health conditions.

• Complete all documentation in compliance with federal/state hospice regulations and HOL policies to ensure regulatory compliance.

• Actively participate as a member of the interdisciplinary team, providing input to support patient/family care objectives.

• Attend team meetings, contribute to care planning, and collaborate with colleagues to coordinate patient/family services.

• Provide consultation and assistance to colleagues, HOL administration, patients/families, and community partners as needed.

• Support level-of-care changes and discharge planning for patients and families.

• Respond promptly to referrals from the interdisciplinary team and community for social services needs.

• Accept assignments consistent with education, experience, and competence.

• Promote teamwork, effectiveness, and efficiency through a cooperative and professional attitude.

 


Education and/or Experience

Required - Master's Degree from an accredited School of Social Work.Required: Master’s Degree in Social Work from an accredited program.Required: Minimum 1 year of experience delivering social work services in a healthcare setting.Required: At least 3 years of experience in inpatient, outpatient, or home-based social work.Required (Hospice): Demonstrated experience in palliative care settings (hospice or related), including: Independent and collaborative work within an interdisciplinary team. Palliative assessment and management of physical, psychosocial, and spiritual needs during aggressive treatment and end-of-life care.Preferred: Knowledge of research techniques, data collection, policy/program/curriculum development.Preferred: Experience with quality improvement methodology.Preferred: Post-degree experience in a hospital or other healthcare setting.


Skills

Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.Other Skills and Abilities:Ability to read, write, speak, and understand English.Demonstrates strong verbal and written communication skills and embraces the integrated healthcare at home philosophy.Proficient in operating standard office equipment and electronic medical record systems.Strong critical thinking, problem-solving, time management, and organizational skills.Promotes patient and family autonomy and self-determination through effective care and support.Skilled in patient and family assessment and care planning.Ability to respond professionally to inquiries or complaints from regulatory agencies, patients, families, or members of the business community.Works cooperatively with a variety of healthcare professionals across diverse care settings.Maintains a professional appearance and attire.Must not appear on the OIG Excluded Providers List.Requires extensive local travel in a personal vehicle to deliver services.Demonstrates flexibility and adaptability in a dynamic, fast-paced environment.


Certifications, Licenses, and Registrations

Required (Home Health Only): BLS (American Heart Association, Vendor Managed).State-Specific Requirements:Washington (WA): Advanced Social Worker License or Associate License; OR Clinical Independent Social Worker License or Associate License (Vendor Managed).Alaska (AK): Master Social Worker License OR Clinical Social Worker License (Vendor Managed). Driving may be required; proof of license and insurance required per policy.Texas (TX): Master Social Worker License (Texas State Board of Social Worker Examiners, Vendor Managed).New Mexico (NM): Master Social Worker License (New Mexico Board of Social Work Examiners, Vendor Managed).California (CA): Social Worker License preferred upon hire.


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
