Social Worker - Mental Health Navigator - Hickman Hospital - Full Time - Days
Primary role is to provide direct patient/family intervention for the delivery of mental health services for patients with behavioral/psychiatric issues in any clinical setting within Bixby and Herrick hospitals. Also provides a wide array of social work services to patients and family to promote understanding and resolution of problems related to environmental stress, physical illness, interpersonal conflicts, and other psychosocial issues. ACCOUNTABILITIES *All duties listed below are essential unless noted otherwise* 1. Interviews patients, relatives, and others to assess personal and social adjustment and determine necessary social intervention. 2. Completes psychosocial assessment histories at the request of physicians, care navigators, or other members of the treatment team and per criteria. Communicates pertinent information regarding the patient’s care to other team members concisely and timely. 3. Coordinates transition planning with the appropriate clinical team and other agencies as appropriate, particularly placement issues. 4. Acts as a liaison between patient, family, physician, and nursing staff to arrange the transfer of patients. Coordinates and leads treatment team meetings to develop and evaluate the multidisciplinary plan of care. Provides for patient’s safety in environment and communication with patients and significant others. 5. Verifies insurance coverage, co-pays, deductibles, etc. When appropriate, contacts clients and providers to obtain insurance information, etc. Follows up on patient insurance coverage and notifying transition facilities as required. 6. Regularly works with difficult and complex cases in coordination with the medical team for purpose of case management, transition planning, and emotional and financial adjustment to illness services. 7. Refers patients and significant others to community agencies for counseling regarding interpersonal conflict and emotional problems. 8. Performs all other job duties as assigned. Education: Master’s degree in social work, guidance and counseling, or similar degree Skills: Must have interpersonal and communication skills to respond effectively and efficiently to critical situations involving patients, personnel, families, and outside agencies in a professional manner. Must have sufficient maturity and emotional balance in order to participate in difficult situations involving psychiatric behaviors, abuse, death, dying, etc. Must have excellent oral and written communication skills. Ability to function in a teamwork environment, considering role as part of a multidisciplinary team. Must be able to address multiple priorities in a timely manner. Demonstrates sound clinical skills and judgment in providing therapy. Must be able to use computer. Years of Experience: N/A License: Current State of Michigan licensure in social work or other counseling. Certification: BCLS certified or obtain within 3 months of hire.
