Job Summary:
The Social Work Director oversees the social services department within a long-term care facility, ensuring high-quality psychosocial support and advocacy for residents and their families. This role ensures compliance with regulatory standards, manages the social work team, and collaborates with interdisciplinary staff to support residents’ emotional, social, and mental well-being.
Key Responsibilities:
Lead, supervise, and mentor the social work team, including social workers, case managers, and activity coordinators.
Develop, implement, and evaluate individualized care plans in collaboration with nursing and clinical teams.
Conduct resident assessments, psychosocial evaluations, and family consultations.
Advocate for residents’ rights, needs, and quality of life.
Ensure compliance with federal, state, and local regulations, including CMS, OBRA, and state-specific LTC standards.
Coordinate discharge planning, community resources, and transitions of care.
Collaborate with families, residents, and the interdisciplinary team to resolve concerns and promote resident satisfaction.
Maintain accurate and timely documentation of all resident interactions, care plans, and regulatory reports.
Participate in quality improvement initiatives and facility committees.
Oversee staff training and professional development within the social services department.
Qualifications:
Master’s degree in Social Work (MSW) from an accredited program.
Current state licensure as a Licensed Social Worker (LSW, LCSW, or equivalent).
