Description

Position Summary:

Functions as the key member of the health care team responsible for ensuring appropriate discharge planning ensured through assessment, (social, financial, and health conditions of patient) from which recommendations for referrals, services, and post hospital care will be provided. This function will be in collaboration with physicians, nursing staff, physical therapy and/or other members of the multi- disciplinary team as appropriate.

Qualifications

Education:

Graduate of an accredited school of professional Social Work (MSW) required. Candidates with a Masters degree in Business (MBA) and/or Health Administration (MHA) will be considered.

Experience:

Three (3) years acute care experience as a Social Worker preferred. 

Certifications:

Required: MSW. Must be currently licensed, certified or registered to practice profession as required by law or regulation in state of practice or policy. Active license for state(s) covered.
Preferred: Accredited Case Manager (ACM)
