BASIC FUNCTION:
Provide social services to patients and/or family as appropriate to meet the needs and treatment goals of the individual. This service would include the development and implementation of social work assessment, treatment, and discharge plans. Provides consultation services as appropriate under Bachelor Level Degree. Participates in individual, group, and family therapy sessions. Responsible for social work services for patients and for families to include all in-patient units and, as needed, E.D., and outpatient departments. Works as a team with the designated RN case manager to complete the case management process.
JOB DUTIES:
1. Provides comprehensive assessments of patients/families needs, as these
relate to episodes of acute and/or chronic illness and crisis situations.
2. Establishing treatment goals and provide alternatives to assist the patient and/or the family.
3. Assesses the needs of all inpatient and observation patients along with the RN Case Manager. Completes discharge planning assessment on high-risk patients.
4. Collaborates with the RN Case Manager and the Palliative Care Team for coordination of discharge planning/utilization management.
5. Acts as liaison between the hospital and available community resources and
participates in conferences, agency meetings, etc. to increase professional knowledge.
6. Collects quality data as designated and participates in the case management outcome analysis.
7. Participates in hospital and/or community committees as appropriate.
8. Performs other related duties, commensurate to skills and training as assigned by the
Case Management Director.
9. Responsible for checking emails and online learning assignments (Relias) on a regular basis.

AGE APPROPRIATE CARE:
Colleague has been instructed in providing customer service and on interacting with age groups as related to the department's general client population: including use of appropriate interventions, communication and skills to match the age and/or abilities of the patient/family.
MACHINES, TOOLS & EQUIPMENT USED:
A. Utilizes the in-house telephone system correctly
B. Demonstrates correct use of computer/software systems
C. Demonstrates correct use of copier and FAX machines
D. Knowledge and use of all case management and hospital forms, records, and reports
E. Knowledge of hospital and departmental policies and procedure manuals and work guidelines.
EDUCATION, TRAINING & EXPERIENCE:
A. Minimum Bachelor’s Degree, preferably in social work from an accredited school.
B. One year experience in social work at an acute care facility is preferred or previous work experience in a social agency or social service department of a health agency.
C. Must have completed Ohio State Social Work Licensure (LSW) or (LPC)
D. Must be computer literate to complete task(s) as required. Such as MediTech (Hospital Support Software), Halogen (performance appraisal system) Relias (education system), ability to retrieve e-mails, complete on line enrollment for health insurance/benefits and any other programs as assigned
PHYSICAL REQUIREMENTS:
A. Able to walk/stand for up to 7.5 hours/shift.
B. Able to bend to reach items on floor, bedside level and stretch to reach overhead.
C. Tall enough to be able to reach over as bed far enough to assist with patient care.
D. Able to lift 50# intermittently, unassisted.
E. Able to see clearly and hear normal speaking voices.
F. Able to communicate verbally both interpersonally and over the telephone in a clear speech.
G. Dexterity necessary to write clearly.
