We are seeking a compassionate and skilled Social Worker to join our team, providing and facilitating patient care in both home and clinical settings. This role operates within established policies and procedures, requiring a high level of independent judgment in planning, delegating care, and evaluating outcomes. The Social Worker will conduct assessments, develop treatment goals, and empower patients to overcome barriers to care. This position does not include clinical social work practice such as diagnosis or psychotherapy.
Key ResponsibilitiesConduct psychosocial assessments of patients, families, and social systemsDevelop and implement strengths-based care plansProvide brief, solution-focused counseling and patient advocacyFacilitate referrals to community resources and coordinate continuity of careConduct one-on-one home or office visits (minimum 30 minutes)Educate patients and families on health topics and available servicesCoordinate with interdisciplinary teams and Medicaid Health PlansDocument assessments, plans, interventions, and outcomes accurately and timelyPromote positive decision-making, stress management, and coping skillsEnsure compliance with applicable policies, guidelines, and lawsUphold the NASW Code of EthicsPerform other related duties as assignedQualificationsEducation
Minimum: Bachelor’s Degree in Social Work (BSW)Preferred: Master’s Degree in Social Work (MSW) from an accredited institutionLicensure
Minimum: Current Michigan licensure as a Licensed Social WorkerPreferred: LMSW (Licensed Master’s Social Worker) with clinical designationExperience
Minimum: 1 year of social work experience with families or equivalent education/experiencePreferred: 2+ years in a healthcare setting; experience in community health, case management, and patient advocacySkills & Abilities
Strong critical thinking and prioritization skillsExcellent verbal and written communicationAbility to manage crises and maintain professional composureKnowledge of psychosocial interventions and community resourcesPreferred: Fluency in SpanishTechnology
Proficiency with standard desktop applications and navigationAbility to use software required for documentation and communicationPhysical & Mental Requirements
