**Job Description** **Job Title:** **Social Worker - Adolescent Health Center (Eating Disorders)** **_Social Work Services at the Mount Sinai Hospital_** The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. The Mount Sinai Adolescent Health Center (“MSAHC” or the “Center”) works to break down racial, social and economic barriers to health care and wellness for young people by providing vital services - high quality, comprehensive, integrated and confidential - for all who come to the Center or school based health facilities, at no cost to them. The MSAHC advances adolescent and young adult health as a national imperative by serving as a leading center of clinical care, specialized training, innovative research and advocacy. The A28 Social Worker will provide social work services to at risk adolescents and their families in an integrated medical and mental health primary care setting at MSAHC. She/he/they will provide short term individual, family, and group psychotherapy, intense case management, crisis intervention, and health education as part of an interdisciplinary medical and mental health service model. A28 SW will consult with medical providers who are working with high-risk patients with both medical and mental health needs and conduct clinical assessments on patients referred by medical providers. These include providers within primary care and child and adolescent psychiatry. All A28 SW’s will participate developing an innovative model to engage and retain adolescents who have a variety of psychosocial needs, particularly focusing on individuals with a history of trauma (e.g. sexual abuse, domestic violence, community violence, physical abuse. As part of an interdisciplinary team, she/he/they will focus on early identification, intervention, and support for individuals struggling with disordered eating and diagnosed eating disorders within a primary care setting. The social worker will provide psychosocial assessments, brief interventions, care coordination, and referrals while working collaboratively with the dietitian, psychologists, and other allied professionals. In addition, the A28 SW’s will also provide assessments for gender dysphoria and hormone and surgery readiness if needed. They will be expected to provide counseling related to medical transition. She/he/they will be charged with ensuring that the above functions are delivered within a culture that is youth-centric, high quality, integrated and collaborative as well as in accordance with Mount Sinai’s policies and procedures, New York State regulations and guidelines, and funder expectations. **Qualifications** + NYS LCSW or LMSW. + Commitment to MSAHC?s mission, vision and approach in order to ensure a patient-centric culture which is collaborative and multidisciplinary. + A passion for working with young people and a work ethic in which a commitment to understanding and solving the needs of each patient is a priority. + A demonstrated commitment to diversity, equity, inclusion and belonging. + Clinical experience within a training program or post training job experience working with adolescents and young adults with psychiatric disorders including eating disorders. + Skillful in assessing patient needs in relationship to health and psychosocial functioning and establishing intervention accordingly. + Significant experience working with low-income, inner-city, at-risk adolescents. + Experience working with populations with complex trauma histories _preferred._ + Experience working with LGBTQIA identified adolescents and young adults _preferred._ + Ability to be flexible and use no traditional approaches in working with young adults _preferred._ + Experience in delivering therapeutic interventions, such as preventative and risk-reduction models that reflect developmentally appropriate methods and materials in working with adolescents and young adults (i.e. TF-CBT, DBT, EMDR, IPT-A, CBT-E, Family-Based Treatment (FBT), and Maudsley). + Excellent collaborative, clinical and communication skills. + Excellent organizational and documentation skills. + Skillful in collaborating with external resources and coordinating appropriate referrals. + Solid working understanding of adolescent growth and development. + Working knowledge of state regulations related to child abuse, neglect, and other urgent care issues. + Bilingual (English/Spanish) _preferred_ . Non-Bargaining Unit, 186 - Social Services - MSH, Mount Sinai Hospital **Responsibilities** Clinical Care + Assessment and treatment planning of patients referred from medical providers in Article 28 setting. + Provide short term therapy up until 1 year. + Consult closely with medical providers ( and other members of the primary care medical team) and legal advocates who are working with patients. + Follow up with referring providers. + Develop therapeutic alliance. This includes youth with history of trauma ( e.g. sexual abuse, domestic violence, community violence, physical abuse) and transgender/gender non-conforming youth. + Consult closely with medical providers and child and adolescent psychiatrists who are working with patients. The aim is to provide coordinated and holistic care. + Collaborate with external resources as appropriate. + Develop effective treatment plans. Treatment plans will be documented yearly in the electronic medical record. + Track treatment progress using validated instruments. + Participate in an interdisciplinary effort that fosters a cohesive culture across staff with improved communication and accountability. + Provide evidence-based treatment for adolescents and young adults with eating disorders, including—but not limited to—CBT-E, Family-Based Treatment (FBT), and Maudsley. + Collaborate with all practices to ensure integration of the MSAHC integrative model of care. Maximize Capacity & Access + Consistently meeting benchmarks and productivity targets. + Documentation as per compliance guidelines. Compliance + Ensure the Standards of Care and Article 28 regulations are adhered to in all aspects of clinical service delivery, reporting, incident review, and high-risk case management. + Knowledge of and ability to implement state regulations related to child abuse, neglect, and other urgent care issues. + Completion and proper documentation of the following: psychosocial assessments, treatment plans, suicide risk assessments, individual and group therapy notes and case management or referral summaries in EPIC the day of encounter. + Adhere to all compliance mandates set forth by all regulatory agencies to include the OMH, DOH, MSHS and the MSAHC; + Adhere to caseload requirements. + Participate in weekly/bi-monthly supervision with A28 Supervisor. The salary range for the role is $85280 - $97760.36 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
