This position will cover the Emergency Department with a schedule of Sunday - Thursday or Tuesday - Saturday Days.

What You’ll Do:
Offer crisis support, counseling, and resource referrals for patients and families
Help plan safe discharges and transitions of care
Coordinate with healthcare teams to keep patients supported and informed
Connect patients to community resources for housing, food, transportation, and more

Job Requirements
Education: Bachelor's (BSW) or Master's Degree (MSW, or willing to enter a Master's Degree program) from a school of social work accredited by the Council of Social Work Education REQUIRED
License/Certification: Social Work Certification REQUIRED
