Social workers help communities get access to the health or social services that they need, improving or providing those services where they are not already in place. They participate in legislation and help families and individuals get access to counseling and psychotherapy when necessary. Social workers are essential social, economic voices and advocates who work on behalf of those facing physical and mental illness, disability, divorce, loss, unemployment, abuse, addiction, and other daunting life challenges. Your work will directly influence patients' lives, helping them move toward stability, healing, and improved overall well-being. 

A caregiver in this position works days Monday – Friday from 8:00 a.m. – 5:00 p.m.

A caregiver who excels in this role will:  

Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.  Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings). Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.  Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.  Maintain a network of resources for consultation and connect patients with community resources.  Supervise Social Work Interns and assist with training new caregivers.  Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.

Minimum qualifications for the ideal future caregiver include:  

Master's Degree in Social Work from a CSWE accredited programCurrent state licensure as a Social Worker (LSW)Licensed Independent Social Worker (LISW) within 5 years of hireTwo years of experience in a hospital and healthcare setting

Preferred qualifications for the ideal future caregiver include:  

Social work experienceEpic experience

Physical Requirements:

Manual dexterity to operate office equipment.May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.Must have normal or correction visionMust be able to clearly communicate verbally by phone.

Personal Protective Equipment:

Follows Standard Precautions using personal protective equipment as required.
