Job Duties and Other Information Job Duties: + Provide clinical supervision of social service staff that perform duties including but not limited to psychosocial assessment, treatment planning, individual, family, and group counseling and discharge planning. + Provide supervision to ensure all work activities are within The Joint Commission (TJC) and Center for Medicare and Medicaid Services (CMS) guidelines. + Coordinate social work services as indicated with other disciplines and agencies. Determine work assignments of staff, ensuring adequate social service coverage is provided. + Interview patients, family members, collateral sources, and obtain and review all available records to formulate comprehensive psychosocial assessments. + Prepare annual updated psychosocial assessments utilizing the same process. + Share and utilize information obtained in assessments as a member of the interdisciplinary treatment team in developing, implementing, and revising individualized treatment plans. + Complete case reviews of risk factors. + Provide individual and family counseling and education.
