We are looking for a Social Worker to provide clinical assessment and treatment of clients with various mental health diagnoses, substance use disorders, medication-assisted treatment, and high-risk patients in crisis. The clinician completes comprehensive biopsychosocial assessments, treatment plans, screenings, and treatment via individual/family/group therapy, as well as discharge plans for patients and families as needed. The clinician works on an inter-disciplinary treatment team in order to provide comprehensive care, as well as continuity of care, and will prepare and send progress reports to the appropriate social service agencies and referral sources as required. Job Duties and Other Information Job Duties: + Completes intake and psycho-social evaluations on primarily adults and some aftercare individuals, which includes providing technical and clinical input on diagnosis and treatment in the interdisciplinary staff consultation with staff prescribers to establish an individual's treatment plan for each individual in treatment. + Completes correspondence, written reports, and other communications with other agencies regarding an individual's care + May provide crisis evaluation for individuals who are in acute distress as indicated by a family member, coroner, law authorities, etc., which may require complex and specialized crisis intervention, psychosocial evaluation, and specialized clinical treatment.
