Position Summary:

Under direct supervision the incumbent identifies and evaluates potential clients as total persons and identifies the functional limitations which impede independent living. This position reports to the Supervisor Care Manager

Minimum Qualifications:

Two years of experience providing social service in one or more of the fields of aging health and long term care services requiredBachelors degree in social work from an accredited school required (BSW)Current CA Drivers licenseBLS Certification from an American Heart Association program requiredAbility to relate to clients and familiesExcellent communication and presentation skillsWhere You'll Work
