**Job Description** **UW Medicine Primary Care at Kent Des Moines** has an outstanding opportunity for a **Social Work Assistant.** **WORK SCHEDULE** + 40 hours per week + Monday-Friday + Day Shift + This position supports our South/East Region- Kent Des Moines, Federal Way, Issaquah, Factoria and Kirkland. 3 days per week are at Kent Des Moines and 1 day per week will be located at one of the other locations within the region. **DEPARTMENT DESCRIPTION** Conveniently located on Pacific Highway South and not far from I-5, UW Medicine Primary Care at Kent/Des Moines offers appointments six days a week, including evenings and same-day appointments. Medical advice is available 24 hours a day, seven days a week. **POSITION HIGHLIGHTS** Serve as a member of a centralized team that is focused on providing supportive social work services to UWPC patients with emphasis on supporting the delivery of patient care and assisting with care coordination, resources, and referrals. **PRIMARY JOB RESPONSIBILITIES** + Provide social work support for Primary and Urgent care + Support and closely coordinate behavioral health care with the patient’s primary care provider, social worker, and UWPC consulting psychiatrists. + Provide patient education about common mental and substance abuse disorders and available treatment options + Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated. Facilitate patient engagement and follow-up in behavioral health care + Track patient follow-up and track clinical outcomes using standard measurement tools and a behavioral health registry + Provide clinical services with respect for patients/family rights and within the NASW code of ethics + Engages in departmental program development activities, tasks forces, and committees. + Performs related duties as assigned/required **REQUIREMENTS** **Social Work Assistant I:** + A Bachelor’s Degree in a behavioral science. + Experience in health care setting or equivalent Ability to obtain Washington State Agency Affiliated Counselor Registration within 60 days of hire. Or **Social Work Assistant II:** + A Bachelor’s Degree in a behavioral science. + Two-years' experience as a Social Work Assistant I or equivalent + Ability to obtain Washington State Agency Affiliated Counselor Registration within 60 days of hire. **ABOUT UW MEDICINE PRIMARY CARE** UW Medicine Primary Care is a network of clinics located throughout the Puget Sound region.
