Job Summary:

We're seeking a dedicated and compassionate Social Work Assistant to join our multidisciplinary team! As a Social Work Assistant, you'll play a vital role in helping our residents navigate the rehabilitation process and achieve their personal goals.

Responsibilities:

Assist social work professionals in conducting assessments, developing care plans, and providing supportive counseling to residents and their families

Identify and address social, emotional, and psychological needs of residents, including those related to dementia, behavioral issues, and end-of-life care

Collaborate with interdisciplinary teams to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of residents

Provide emotional support and crisis intervention to residents, families, and staff as needed

Maintain accurate and confidential records of resident interactions and care plans

Participate in staff meetings, training sessions, and other educational opportunities to stay current on industry trends and best practices

Qualifications:

High school diploma or equivalent required; associate's or bachelor's degree in social work, psychology, or a related field preferred

1 years of experience in social work or a related field

Basic knowledge of social work principles, theories, and practices

Excellent communication, interpersonal, and documentation skills

Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines

Strong emotional intelligence and empathy for residents and families
