No County Franklin Street Address 2445 State Route 30 City Tupper Lake State NY Zip Code 12986 Duties Description PRIMARY DUTIES AND RESPONSIBILITIES: • Performs a wide variety of social services and support for assigned individuals. • Independently provides written, verbal assessments, and service programs and provides written and oral input to treatment team meetings.• Monitors individual’s programs to ensure continuity, consistency and progress as determined by the Interdisciplinary Treatment Team as related to the social work scope of needs.• Responds to emergency situations as needed. • Attends and participate in treatment team meetings. • Completes admission and discharges of individuals which includes required paperwork and follow-up. • Completes and submits, 30-day, quarterly and annual assessments in advance of treatment team meetings. • Attends and provides input and assessment at individual’s treatment team meetings.• Provide individual counseling sessions as needed.• Ensures and provides documentation of services delivered, including interventions, reflecting needs, strengths, goals, level of participation and rights.• Advocate for the needs of the individuals. • Coordinates and arrange family visits. • May accompany individuals on family visits, and must complete all home site assessments.• Obtain, monitors, and follow-up on consents for RRC and ICC in a timely manner ensuring that they are current and complete. • Functions as a liaison between individual’s families and the team.• Obtains informed consent from individuals/families/legal guardians for any rights/restrictions or restriction intervention. • May teach classes.• Complete and upload MMAs and PEPs.• Compile LRAC/SFAC packets and community referral packets as needed for individuals. • Assist with self-advocacy group.• Other duties as assigned. Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status.
