Salary: $1720 / Week $43/hr. Amergis Healthcare Services is actively looking for a SocialService Worker who would be interested in a 13-week, Full-TimeContract in Chicago, IL. Additional details of thecontract are below: Position: Social ServiceWorker (LPC/LSW or higher) - Will be on the road conducting client visits 4 out of the 5days in the week. Will be in office once a week Location: Chicago, IL (Mainlywest/south side) Pay Rate (Local): $43/HR Schedule: · Mon-Fri · 9am – 5pm (Needsto be flexible) · Occasional clientvisit on a weekend · 40 hr./wk. Duration: · 13 Weeks (LikelyExtension) · Possible Temp toPerm Requirements: · LSW or LPC – will also accept LCPC or LCSW · Has a valid driver’s license, car insurance, and a car · Able to float throughout Chicago and Chicagoland area throughoutthe day to different families/clients (mainly south and west side of Chicago/suburbs) · 3-6 client visits a day · Detail orientated · Good note taker · Comfortable working with people of different backgrounds · Comfortable meeting with clients in the client’s homes,public settings, and court houses · Make sure that the families are being taken care ofproperly Duties/Responsibilities: · Interviews potential clients in agency offices or bytelephone; obtains background information on client and determinesappropriateness of request for service. · Carries a selected caseload with meeting the basicenvironmental and psycho-social needs of the client. · Performs a variety of casework duties, including obtaining,analyzing and evaluating data related to cases. · Implements plans of action for cases, following establishedprocedures. · Formulates and frequently reviews recommended handling ofcases with the supervisor. · Implements the specified treatment process, analyzing andevaluating current status of clients. · Make frequent reports to the supervisor on the progress ofeach case and performs other formal record-keeping functions required by theagency. · Studies agency policies and practices regarding caseworkand other functions and participates in other areas of the casework process,such as intake.
