The Chenango County Cooperative extension is seeking two individuals, who enjoy coordination work and helping community members. The Social Care Navigation position is an amazing opportunity for someone who has some social work, human services or healthcare experience. Even though this is a temporary assignment at this point in time, the individuals will be eligible for the NYS benefits, which includes the health insurance, free dental and retirement. The goal of this position is to support community members facing a range of challenges to connect to resources that can address their social needs and improve their health and well-being. The specific function is to support CCE-Chenango’s participation in New York’s Medicaid 1115 Waiver through provision of screening and navigation services for Medicaid members in the Southern Tier region. The Social Care Navigator will 1) identify Medicaid members’ unmet social needs by using a standardized screening tool, 2) complete an Eligibility Assessment to determine whether they are eligible for Enhanced Services through the region’s Social Care Network, and 3) provide navigation services and ongoing care management as appropriate based on screening and eligibility assessment results.
The Social Care Navigator will work closely with Care Compass Collaborative to serve Care Compass Network's primary care practices. This position provides an excellent opportunity to gain experience with local anchor institutions in the areas of health and social work.
+ Required Qualifications
+ Computer Skills including use of e-mail and computer based time collection, and willingness to learn new referral management platforms (primarily Unite Us, which is the chosen platform of the Southern Tier region for 1115 Waiver-related work) .
+ Associate’s Degree or high school diploma with 2 years transferable program/functional experience may substitute for degree requirement.
+ Ability to work independently with a minimum of supervision and to stay organized to complete work within a reasonable time frame
+ Experience working in a team-oriented, collaborative environment
+ Ability to rapidly adapt and respond to changes in environment and priorities
+ Ability to identify and address safety concerns in the work place
+ Ability to work effectively with co- workers
+ Ability to communicate with supervisor and staff as pertains to the duties of the job.
+ Ability to follow oral and written instructions and assume responsibility with general guidelines and direction
+ Ability to plan and organize work responsibilities according to priorities developed with the immediate supervisor
+ Ability to meet travel requirements associated with this position
+ Ability to work flexible hours which may include evenings and/or weekends, as appropriate

Preferred Qualifications:
+ Experience with community members facing challenges
+ Familiarity and interest in with 1115 Waiver goals and procedures
+ Familiarity with trauma-informed approaches and interest in advancing health equity through cross-sector collaboration and health and social care integration
+ Experience managing a caseload of clients and providing 1:1 care management support
Responsibilities:
+ Deliver screening and navigation services in accordance with 1115 Waiver requirements and any other services mutually agreed upon with their supervisor
+ Complete timely outreach and provision of social care navigation services in coordination with team members
+ Assist in the development of resource navigation services to meet identified needs and diverse interests of participants
+ Coordinates referrals platform responsibilities t o provide services to clients and initiate invoicing for services rendered. Keep necessary records
+ Assist Supervisor in making programmatic changes based on program evaluation outcomes and to meet changing local and/or statewide priorities or needs
+ Assist in the interaction with program participants and collaborators, Cornell faculty, and staff to collect evaluation data for improving programs
+ Assist Supervisor with interpreting evaluation data for reporting and program improvement purposes using standard, established policy, procedure and methods
+ Assist with recommending program changes to strengthen and improve programs
+ Other duties as assigned
P osition Details:
Expected Weekly Hours: 37.5 hours per week
Hourly Wage: $21.75 per hour
Location: CCE Chenango County office in Norwich, NY
