The Sleep Navigator is a clinical liaison role requiring strong organizational, communication, and problem-solving skills. You will coordinate care for sleep-disordered patients, working closely with physicians and the Sleep Center team to ensure a seamless patient experience from referral through treatment.
Core Responsibilities:Patient Coordination:
Receive referrals, schedule follow-ups, and ensure all diagnostic tests are completed.Contact patients for missed appointments and assist with barriers such as insurance, transportation, and language.Serve as a liaison between patients and physicians, ensuring smooth communication of test results and care plans.Prepare result letters, schedule follow-up appointments, and update patients and physicians on care outcomes.Collaboration & Support:
Work with the Sleep Medicine team to coordinate patient care and improve outcomes.Use evidence-based practices to maintain best practices in sleep medicine.Assist in patient education and help develop educational materials in collaboration with the Marketing Department.Information Management & Performance Improvement:
Maintain patient records and ensure timely communication of information to relevant staff.Participate in performance improvement initiatives to optimize care quality and efficiency.Customer Service & Engagement:
Address patient, staff, and physician satisfaction by identifying areas for improvement and creating action plans.Assist in team building and staff education to support a positive working environment.Required Qualifications:Education:
Associate’s Degree in Sleep Technology or related field required.Licenses & Certifications:
Certified Clinical Sleep Health (CCSH) required.Registered Polysomnographer or Registered Sleep Technologist required.Basic Life Support (BLS) from American Heart Association required.Experience:
Minimum of 4 years of clinical experience in sleep medicine in an AASM-accredited facility.Required Skills:
