Acute Care team at Hartford HospitalThis position is open to Physical Therapists Occupational Therapists and Speech Language PathologistsThis leader will be a part of a larger dynamic leadership team and an experienced team of clinicians that brings decades of experience in providing the most innovative careOverviewIn addition to direct patient care responsibilities the Site Supervisor supervises the operational activities of the Acute Care team and collaborates with Rehab and Hospital Leadership on the management of patient care day to day operations and department personnel They are responsible for staff development hiring supervising and evaluating team membersThe Site Supervisor assists with supply management policies and procedures patient scheduling patient and staff safety operational monitoring regulatory compliance employee engagement and the overall timeliness and quality of services provided to customersThe Site Supervisor participates in multiple committees across the hospital representing Rehab They are responsible for metrics reporting and dashboards as well as referral management PSAG and other patient care and safety huddles Manages all DPH and Joint Commission auditsAdditionally approximately of time is spent in a direct patient care capacitySupervisory Responsibilitiesmiddot Responsible for coaching training onboarding employee engagement staff rounding timekeeping onboarding new staff and performance evaluations Leads daily management huddle boardsmiddot Delegates patient care tasks Provides clinical supervision in area of specialty as requiredmiddot Actively participates in enhancing patient experience quality control and process improvement initiatives Ensures regulatory compliance and assists with incident resolution policiesprocedures and emergency preparednessmiddot Identifies opportunities for improvement recommends process improvements utilizing operating model and takes necessary steps to improve processesmiddot Responsible for managing all DPH and Joint Commission audits Makes sure all staff are compliant with education all files are reviewed responds to all inquiriesClinical Responsibilitiesmiddot Completes all documentation including but not limited to evaluation care plan and goals patient status and progress visits patient discharge summary and patient care transition plan and summarymiddot Participates in patient transition planning andor patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care Helps to coordinate patient care with other care providers such as Rehab Therapists Nurses Social Workers and other community resources as related to care transition planningWork whereevery
