As a Hospital Care At Home (HCAH) Service Coordinator, you will coordinate the logistics for supplies, equipment, and services related to the Hospital Care at Home (HCAH) patients, routinely interfacing with medical care providers, labs, supply and testing facilities ensuring the needs of the patient are met as needed and on time. You will ensure that these patients have everything they need for at-home care including IVs, imaging, labs, food, and social support all from within the command center. This is a fast-paced environment, requiring strong problem-solving skills. As the team grows, you will have the opportunity to pursue leadership opportunities across the hospital network. You will also gain a unique avenue to ensuring quality patient care, helping caregivers meet them in the comfort of their own homes. **A caregiver in this position works rotating days and weekends from 6:00AM - 2:00PM.** A caregiver who excels in this role will: + Collaborate between physician offices, patients and clinical/non-clinical departments to ensure a seamless model of access and care benefiting patients, physicians and family members. + Coordinate, schedule, dispatch, monitor trends, manage, track and report with respect to the delivery and performance of Acute Rapid Response Services as ordered by HCAH Command Center Providers. + Ensure equipment delivery, installation and deinstallation. + Coordinate lab tests ordered by HCAH Command Center Providers and supply delivery from order to service fulfillment. + Provide information and trend to the care team to identify and resolve barriers to patients plans of care. + Work with patients and families to schedule ordered services. + Collaborate and communicate with HCAH Command Center Providers, Patients, MHG Contracted and Cleveland Clinic Florida Home Providers and out-of-home service providers to schedule the delivery of services, equipment and/or supplies, schedule necessary transportation and confirm the delivery of services, equipment and/or supplies. + Assess satisfaction and/or address concerns or complaints to managers and provide administrative and logistical support. + Attend regularly scheduled and as needed departmental meetings and events. + Question policy and procedure when appropriate, provide solutions to problems and elevate concerns as needed. Minimum qualifications for the ideal future caregiver include: + High School Diploma or GED + One to three years of registration, scheduling, billing, coding, healthcare or physician office experience + Two to five years of face-to-face customer service experience + Proficient in the use of Microsoft office products and other applications + Knowledge of medical terminology + Ability to compose written procedures, schedules and reports + Excellent communication skills + Ability to multi-task and manage multiple simultaneous deliverables Preferred qualifications for the ideal future caregiver include: + Background in 9-1-1 dispatch **Personal Protective Equipment:** + Follows standard precautions using personal protective equipment as required **Pay Range** Minimum hourly: $17.25 Maximum hourly: $26.31 The pay range displayed on this job posting reflects the anticipated range for new hires.
