Description
We are seeking a Service Coordinator to manage day-to-day operations, act as a liaison between clients and providers, and oversee a multidisciplinary care team, including CNAs, Drivers, and Day Habilitation Specialists. This role ensures high-quality, client-centered services while complying with industry standards and organizational policies.
Key Responsibilities:
+ Coordinate schedules and services, maintain effective communication with clients, caregivers, and staff, and ensure service alignment with client needs and goals.
+ Supervise care staff, conduct performance reviews, and manage team collaboration and service quality.
+ Handle administrative tasks including scheduling, record-keeping, billing, and maintaining compliance documentation.
+ Collaborate on process improvements and contribute to the agency’s mission of empowering clients to live independently with dignity.
Requirements
+ High school diploma (Associate's degree or higher preferred in Human Services, Healthcare, or related field).
+ 1-2 years of experience in service coordination or a similar role, preferably with vulnerable populations.
+ Strong communication, organizational, and multitasking skills, with proficiency in Microsoft Office and electronic records systems.
+ Knowledge of HIPAA, a valid driver’s license, and the ability to pass background checks.
