Duties Description POSITION DESCRIPTION: The Senior Licensed Practical Nurse 2 (SLPN2) will operate under the direction of the Hospital Liaison in a full-time capacity. The position will be based out of Neighborhood IRA.PRIMARY DUTIES AND RESPONSIBILITIES:Duties include, but are not limited to:• Conduct visits to nursing homes and rehabilitation centers to report on the care and progress of admitted patients.• Evaluate hospitalized patients to identify necessary care and monitor the progress of their conditions.• Manage supplies, including inventory management, ordering, and organization of resources for the successful discharge of patients back to IRA.• Prepare and organize necessary documentation for external medical appointments related to patient discharge to IRA.• Perform regular chart audits and ensure documentation accuracy in the Electronic Health Record (EHR)• Attend rounds for all individuals admitted to hospitals and rehabilitation facilities, reporting on their conditions, ensuring adherence to care plans, and identifying any emerging needs.• Facilitate effective communication among care team members to promote continuity and quality of care.• Uphold a clean, safe, and organized clinical environment in alignment with infection control and safety standards.• Collaborate with interdisciplinary team members to ensure holistic, person-centered care.• Assist with incident reporting, documentation, and follow-up actions to enhance safety, compliance, and quality improvement efforts.• Utilize effective communication between the IRA’s residential staff and the Registered Nurse. To accomplish this the SLPN 2 will routinely communicate with the Placement Coordinator regarding return to the IRAs for hospitalized or rehabilitating individuals outside of our system of care. • Engage in quality improvement initiatives and support compliance with internal audits, policies, and performance benchmarks.• Assess patient responses to treatments and medications, promptly relaying any changes in health status to assigned RN/physician.• Address concerns raised by individuals, families, advocates and staff, escalating issues to higher-level personnel as needed.• The SLPN 2 will supervise a minimum of one Sr.LPN 1 to begin with. This assignment may expand over time. The supervision of LPNs frees up the RNs to provide the care of the individuals served.• Other duties as assignedNOTE: You must have a valid driver’s license to apply as travel is a part of the duties involved in this position. Minimum Qualifications MINIMUM QUALIFICATIONS:Permanent Qualifications: A license and current registration to practice as a practical nurse in New York State and two years of experience providing nursing care and treatment services to individuals under the supervision of a registered nurse; assisting with the implementation of treatment plans; administering, observing, and recording medication management; educating and counseling individuals and families regarding illnesses, medications, tests, and treatment goals and procedures; monitoring and recording individuals progress; and supervising direct care staff.Transfer Qualifications: Candidates must be eligible for transfer under Section 70.1 of the Civil Service Law.To determine if your current Civil Service title is eligible for transfer to this title, visit the Career Mobility GOT-IT website: https://careermobilityoffice.cs.ny.gov/cmo/gotit/index.cfm.
Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.• Please note that starting salary for this title reflects the increased hiring salary, geographic pay differential and Mid-Hudson adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for CSEA Grade 13 ($53,413), Increased Hiring Salary ($6,656), Geographic Pay Differential ($13,000) and Mid-Hudson Adjustment ($2,000). The Geographic Pay Differential and Mid-Hudson adjustment is added to the job rate of ($65,061). • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.• For CSEA positions: Local Bidding Agreements will be honored.• Preference will be given to OPWDD employees impacted by closures.
