The Senior Home Health Field Clinician Educator is the clinical resource responsible for the development of clinicians as they apply knowledge obtained from training into the Home Health environment while following an established orientation model process. He/she instructs and counsels while acting as a role model, leader, influencer, advocate, evaluator, and socialization agent. The Senior Home Health Field Clinician Educator is accountable for the growth and retention of new hires.
**Essential Functions:**
+ Teaches, mentors, and coaches clinicians.
+ Demonstrates competence in performance of clinical skills including but not limited to admission of patients, assessment, and development of care plans, case management, coordination of care and other clinical care duties.
+ Oversees and supervises orientation of new hires in partnership with the branch leaders and patient facing clinicians (preceptor/peer mentor).
+ Models regulatory compliance and standards.
+ Provides oversight and education to ensure effective collaboration with healthcare providers in accordance with physician’s orders and plan of care.
+ Acts as a liaison and maintains communication with new hires, peer mentors, branch leaders, and the Home Health Learning Design & Education team.
+ Ensures execution of orientation experience via the orientation toolkit, initiates competency assessments, and state specific requirements as needed.
+ Continually works to create a culture of learning and support for clinicians as they are gaining confidence and competence in their respective role.
+ Responsible for established KPIs.
+ Assists with education to clinicians with mergers/acquisitions.

**Required Skills/Experience:**
+ RN (BSN preferred) or licensed Physical Therapist.
+ Unrestricted RN or PT license in work state assigned.
+ 2 or more years of home health experience within the past 5 years.
+ Prior nurse/therapy educator experience preferred.
+ Is competent in the delivery of patient care, consistent with evidence based practice and regulatory requirements.
+ Demonstrates strong leadership skills and the ability to function with minimal direction.
+ Has excellent written and verbal communication skills.
+ Displays effective interpersonal and sound decision-making skills.
+ Is organized and committed to timely work completion.
+ Has expertise in Homecare Homebase (HCHB), including PointCare functionality.
+ Can navigate back-office workflows and reports in HCHB R2 preferred.
+ Takes a proactive approach to problem-solving, is innovative and a critical thinker.
+ Is comfortable working independently and collaboratively in a remote environment.
+ Is technologically adept: able to manage multiple screens, facilitator guides, and multitask effectively.
+ Conducts thorough documentation audits and delivers constructive, actionable feedback.
+ Demonstrates proficiency in OASIS accuracy; OASIS certification preferred.
+ Has instructor led teaching experience utilizing virtual platforms: Zoom/Teams preferred.
+ Is proficient with Microsoft Office Suite: Outlook, Teams, Excel, PowerPoint, Word, SharePoint preferred.
+ Possesses effective teaching skills and desire to mentor and develop clinicians.
+ Creates a nurturing and supportive environment favorable to learning.
+ Has strong assessment skills to identify learning needs and provides immediate and thoughtful feedback to the new clinician.
+ Provides effective communication to the clinician/student and leaders in the location supporting performance and progress through orientation.
+ Has a working knowledge of healthcare laws and regulations including state/local specific.
+ Current CPR certification.
+ Travel requirements: less than 10%.

**Additional Information:**
To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
+ At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested.
+ Satellite, cellular and microwave connection can be used only if approved by leadership.
+ Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
+ Home or Hybrid Home/Office employees will be provided with telephone equipment appropriate to meet the business requirements for their position/job.
+ Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

**Pay Range**
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$86,300 - $118,700 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
