As a part of Change Grow Live team you’ll help people get to where they want to be and make a difference in their lives.  

 

Change, Grow, Live services and staff will provide wraparound support to rough sleepers and will work directly with emergency accommodation to support the staff with this group of people.

 

We’re looking for a compassionate and proactive individual to join our multi-disciplinary team as a Senior Complex Needs Navigator. In this vital role, you’ll work closely with street-based services and healthcare partners in Barnet to engage individuals facing homelessness, rough sleeping, and substance use—often alongside physical or mental health challenges.

 

You’ll manage a caseload of people with multiple and complex needs, using a personalised and assertive approach to build trust and encourage access to drug and alcohol services. Your work will help individuals move away from rough sleeping, begging, and anti-social behaviour, supporting them on their journey toward stability and recovery.

This role requires flexibility, resilience, and a genuine commitment to making a difference in the lives of those who are hardest to reach.

 

Please note some shift work including early and late shifts will be required

 

Where: Barnet

Hours: Full time, 37.5 per week

Full Time Salary: £32,002.35 - £34,214.21 pro rata for part time hours (depending on experience) 

Allowances: £2195.72 Outer London Weighting, Pro Rata for part time hours

Contract Type: Fixed Term until 31st March 2026, possibility of extention with based on funding.

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the Role:

Lead a proactive, solution-focused approach to support entrenched clients.Conduct street outreach shifts at varied times and locations to identify and engage rough sleepers and beggars.Provide assertive outreach to individuals unwilling to engage with services.Manage a caseload of complex clients and develop personalised, recovery-focused action plans.Assist homeless clients with timely hospital discharge and accommodation placement.Collaborate with drug and alcohol teams to ensure access to treatment.Use motivational interviewing and other models to support clients with substance misuse.Work with mental health agencies to ensure appropriate support is available.Liaise with housing teams and providers to maximise accommodation opportunities.Deliver training and advice to organisations on rough sleeping and related social issues.Participate in statistical analysis and street counts for monitoring programmes.Work flexible shifts, including unsociable hours and severe weather conditions.Provide backup support for leadership, including strategic meetings, rota management, and duty manager responsibilities.Deputise for the Community and Outreach Team Leader when required.

About you:

Experience working in health and social care with vulnerable clients, including those with substance misuse and mental health needs.In-depth knowledge of at least one area: substance misuse, alcohol misuse, or mental health.Understanding of clients with multiple and complex needs.Awareness of multi-agency working within a strengths-based, asset-building approach.Experience facilitating multi-agency meetings.Proven ability to work in partnership with statutory and voluntary agencies.Strong administrative, recording, and reporting skills.Knowledge of safeguarding procedures for children and vulnerable adults.Familiarity with Supported Housing.Experience in case management.Excellent communication, negotiation, and advocacy skills.Emotional resilience and a pragmatic approach to managing conflicting priorities.Confidence in building relationships and representing the organisation across services.Ability to work independently and collaboratively within a team.Patience and confidence when working with disengaged or challenging clients.Willingness to work unsociable hours and in severe weather conditions.
