Responsible for scheduling clients Home Health/Hospice services with clinician continuity. Identifies client need and schedule clinicians with the appropriate competencies to match client level of care. The Scheduler is expected to exercise judgement in determining the most effective and efficient method of scheduling and rescheduling patients. Provide optimal levels of communication with patients, clinicians, clinical managers, physicians, and homecare coordinators. May also provide coverage for multiple sites and/or Hospice support.

High School diploma required with equivalent 2-4 years industry experience; Associate's Degree preferred2-3 years home care, hospice experience preferredMicrosoft computer skill set required; experience with Trinity Health At Home software platforms and applications preferredWorking knowledge of standard office equipment and remote technology requiredExperience multi-tasking on multiple computer systems..Epic experience a plusMust have valid Driver’s license and reliable transportation to and from work site.Possible Opportunity to work from home.
